New Oklahoma City Ordinance Language

Hey everyone! As many of you know, the
Oklahoma City License is being updated. I just wanted to break the
licensing process down for you as to the best place to start, what fees
to expect and such. Previously we understood that the effective date of
the new law was to be
May 1st but I recently found that the date is
May 28th. Keep in mind though that if you are grandfathering in
you MUST be licensed before the effective date of the new law!
The best place to start this process is at
the
Oklahoma City/County health department at 921 N.E. 23rd phone
405-427-8651. Here you will pick up your licensure application
packet. It includes specific directions for completing the packet. Just
take it with you and complete it as you turn in three packets (included)
but need to attach copies of your O.S.B.I. background check, drivers
license, School transcript (if applicable), applicable work history (I
just attached a resume) and two passport style front faced photos. All
copies must be notarized (mine were notarized for free at my bank but
notary fees are not excessively high).
Before you return your application obtain
your two front face photos ( I had mine taken at
Walgreens for about $8:00)
Visit the Oklahoma State Bureau of Investigation at 6600 N. Harvey phone
405-848-6742 to obtain your O.S.B.I. background check (the fee
here is $15.00). And have three copies of this as well as your drivers
license, transcripts and work history to turn back into the Health dept.
All copies notarized on the signature page of the applications.
Return these materials to the City/County
health dept. I was told that a representative would contact me to do a
facility inspection, but this didn't happen. Simply call the health
dept. after turning in your applications and request Mike Rocky's office
and request an inspection. An inspector (typically Mike Rocky) will
schedule a time with you for the inspection. And if your facility is
approved, he will sign off on the facility.
You can then take the two copies of the
application that he left with you (one is retained by the health dept.)
and your inspection approval papers to the licensing office at 420 W.

Licensing fees;
Manager-$30:00
Operator- $24:00
Outcall- $30:00
Since I operate from a home office, I
needed to purchase the managers license as well as the operators license
(the outcall license will not be available until after the effective
date of the new law. We are still licensing under the existing
ordinance). If you are operating from a spa/salon, clinic, etc. The
owner of the facility applies for the managers license and you just need
to apply for the operators license. Any questions as to this, refer to
the above phone number and request Bonnie Gaynor's office. I ran into
complications as to operating from a home office which Bonnie
straightened out for me.
From here you will take the paperwork
given to you at the Licensing division to the police headquarters on
Colcord, (the licensing agent can give you the directions and it's
within walking distance.) Enter the west side of the building at the
door labeled "fees and permits" and not the main entrance on the south.
Here you will receive "badges" at $7:50
each (this takes a little while so feed your meter if using one to
park). In my case I received a "managers" badge for that license and an
"operators" badge for that license. After the effective date of the law
I will return to obtain the outcall license and a badge for that one.
I hope that this helps to expedite the
process for you. And remember to save your receipts for taxes as
licensure expenses!
